The decision
Separate the person who writes a change from the person authorised to release it. Keep a visible current version and a documented emergency update route.
Rehearse this before accepting the change
Withdraw approval during editing; the draft must not replace the released procedure.
Use fictional examples and an authorised test environment. Record what happened, the evidence you inspected and any issue that still needs a responsible owner.
Choose a useful measure
Count procedures with an accountable approver and unresolved draft changes.
Agree the definition and baseline with the team doing the work. Review the result over a comparable period and note other changes that could affect it.
