Write a file-upload instruction that names the document

An enquiry form permits attachments but says only upload file. Visitors send unrelated reports or screenshots because they cannot tell what the team needs at this stage.

What to change

Describe the specific document and its purpose wherever uploading is an approved part of the process. Avoid using a general attachment option as a substitute for clear instructions.

A worked example

Illustrative example

A referral enquiry asks for the referral document the team specifically requested and explains how to ask for help if it is unavailable. It does not invite a complete personal record collection.

How to check the result

Visitors can identify what to supply and what to leave out of the upload.

A mistake to avoid

Do not ask for additional identity or clinical documents merely because the form can accept multiple files.

Your action checklist

Use these checks to prepare a discussion with your team or supplier. Tick a check when you have recorded the decision or evidence, rather than when a feature has simply been promised.
